RN - Chronic Care - Rural Health Care

The Registered Nurse (RN) Chronic Care Manager (CCM) is a clinic-based position responsible for managing patients who have chronic health conditions, working towards better health outcomes for those patients, and maintaining routine contact regarding their ongoing healthcare needs. The CCM RN supports high risk and chronic conditions patients in collaboration with other team members. He/She will provide services such as patient education, self-management support, coordination of care, and acting as a patient advocate. The CCM RN works as an extension of clinic providers by establishing and maintaining a more centralized coordination of care.The RN, in general, usually does not provide direct patient care, however, he/she will assist in other clinicduties as needed, which may include delivery of direct care.The RN functions under the direction of the Rural Health Clinic Manager and/or the Medical Provider(s) andexercises independent judgment in applying professional knowledge required to plan, prioritize and organizethe delivery of quality patient care coordination.

This position is safety sensitive and subject to WMRMC’s Drug & Alcohol Policy.

Full-Time With Benefits

Required Education
Graduate of an accredited school of nursing

Required Experience
Knowledge regarding the nursing process and its application, including the assessment, planning,
implementation and evaluation of care
Working knowledge of medications frequently prescribed in chronic care

Preferred Education and Experience
Bachelors’ of Science Nursing
Two (2) years previous RN experience in patient care setting
Prior experience working with chronic conditions patients
Prior experience with care coordination
Ability to understand and interpret statistical data related to patient care and outcomes
Experience in Continuous Quality Improvement (CQI)

Required Licenses & Certifications
Valid registration as a Registered Nurse in the state of Arizona
Certification within 30 days of hire as listed below:
Basic Life Support (BLS) is required
