Please use the "Authorization for Release of Medical Information" when a patient would like a copy of their medical record. The patient must provide photo identification and sign the form at the Medical Records Office, which is located in the south wing of White Mountain Regional Medical Center. Outpatient registration personnel can direct the patient to the specific location.
